Starting care is simpler than most people expect. Here is exactly what happens from your first phone call to your first appointment.
Five steps to your first appointment
1. Reach out
Call us or send an inquiry. Our admissions team answers your questions and explains how treatment works — no pressure, no judgment.
2. Verify coverage
We check your insurance benefits, or review self-pay options if you are uninsured, so you know what to expect before your first visit.
3. Schedule your first visit
We find an appointment at the location that works best for you: Columbus, The Plains, or Waverly.
4. Assessment and plan
At intake you complete paperwork, meet our clinical team for an assessment, and build a treatment plan with your provider.
5. Begin treatment
When appropriate, medication and counseling can begin right away, with follow-up visits scheduled around your progress.
What to bring
- A state-issued photo ID
- Your insurance card, if you have one
- A list of medications you currently take
- A routine, judgment-free urine sample is collected at the visit

How do I start the intake process?
You can call Southwestern Recovery Center or submit an inquiry through our website. Our admissions team will walk you through the intake process, verify insurance if applicable, and help schedule your first appointment.
How long does intake take?
Plan for your first visit to take longer than a routine appointment. It includes paperwork, an assessment with our clinical team, and time with a provider to build your treatment plan.
What should I bring to my intake appointment?
Please arrive on time with a state-issued ID, your insurance card, and a list of any medications you are currently taking. As part of your visit, you will be asked to provide a routine, judgment-free urine sample to help us tailor your treatment plan to your needs.
Do I need a referral?
No referral is required. You can contact us directly and our admissions team will help you get scheduled.
Is my information confidential?
Yes. Your care and your records are confidential and handled in line with federal privacy protections for substance use treatment.
